Request or Request For: Which One Is Correct?

Unlike its noun form, the verb request is never used with the preposition for. It is a transitive verb and therefore it is simply followed by a direct object:

The first step in becoming certified is to request an application form.
The first step in becoming certified is to request for an application form.

You must complete this form in order to request a loan.
You must complete this form in order to request for a loan.
